Social Security Numbers and Dates of Birth are required for all covered Spouses and children.

What to expect during your enrollment call: You will speak with a Benefits Counselor on a 1-on-1 basis and have the opportunity to ask questions to make informed decisions about which coverages best suit the needs of you and your family. You will also have the opportunity to include your spouse on the call. Please note that all calls are confidential.

Employees who are waiving medical coverage: must complete a medical waiver form to be returned to HR within 30 days. The form can be found in the Additional Resources tile below.

Dependent verification must be returned to HR within 30 days.

High-Deductible Health Plan enrollees: the HSA form only needs to be completed at the start of the calendar year or if you would like to make changes to your contributions. The form can be found in the Health Savings Account tile below.

Address changes: Employees must complete the Residency Certification Form. This form can be found in the Additional Resources tile below. Forms must be returned to HR within 10 days.
